Student Medical Info

Medical information is required for our Overnight/ Water Excursion/ Activity Consent and Medical Information form.

This form is located in our Forms and Notifications page, which can be accessed under Our School > Forms & Notifications or from the large purple navigation button on our home page. On this page move to the bottom right and select the Secondary Student Medical Info Form. This is an online Google Form that is very easy to complete and submit.

This form must be completed annually and will cover medical, dietary and swimming ability details for any applicable school sport activities and excursions for the year 2020.

Permission notes to attend the activity or excursion will still be required to be submitted each time. Once this form has been completed, any updated information must be submitted to the front office in writing via email or in person.

This form must be completed before submitting the sport permission note due at the end of Week 8, Term 4 2019.
